AuthentiCare Health Centers in Kentucky is seeking a compassionate LPN to join our caring family in long-term, sub-acute, and memory care. You will assess residents, administer medications, and collaborate with a multidisciplinary team to deliver resident-centered care.

Qualifications

• Licensed LPN in Kentucky (state license required).
• Experience in long-term care settings preferred.
• Proficient in medication administration.
• Familiar with PCC EMR (Point Click Care).
• Knowledge of MDS assessments for long-term care.
• Experience in memory care units helpful.
• Strong background in case management.

Responsibilities

• Assess resident health problems and develop nursing care plans.
• Administer medications and treatments as prescribed.
• Monitor resident progress and respond to changes in condition.
• Collaborate with multidisciplinary teams to provide comprehensive care.
• Educate residents and families about health conditions, treatment plans, and discharge.
• Maintain accurate resident records and documentation in compliance with HIPAA and regulations.
• Participate in case management activities to ensure continuity of care.
• Provide support in specialized areas such as PT, OT, SUD, Pain Management.

RESPONSIBILITIES

Including, but not limited to:

• Maintain accurate resident records and documentation in compliance with state and federal regulations, while ensuring compliance with HIPAA regulations.
• Participate in case management activities to ensure continuity of care.
• Provide support in specialized areas such as PT, OT, SUD, Pain Management, etc.

SKILLS

• Registered or Licensed Nurse in the state
• Medication administration
• EMR: PCC (Point Click Care)
• Proficiency in memory care techniques and protocols.
• Knowledge of MDS (Minimum Data Set) assessments for long-term care facilities.
• Familiarity with operations in Skilled Nursing, Sub-Acute, and Dementia Units.
• Strong background in case management practices.
• Ability to work effectively in a medical office environment.
• Excellent interpersonal skills for effective communication with patients, families, and healthcare teams.
• Strong organizational skills to manage multiple tasks efficiently.
