When a resident is admitted to our home, our skilled team of trained professionals meets with the resident and family member, providing a unique and highly personalized plan of care. Our residents are continually assessed for changes to optimize their function and quality of life. Our plan of care always reflects the residents’ individual and personal choices.

Responsibilities:
  • Assist with resident assessment and implementation of care needs and will participate in resident and family care conference.
  • Must be able to effectively communicate with residents, families, staff, physicians, outside providers, and the public.
  • Ensure that changes in resident condition are evaluated, monitored, documented, and effective interventions are implemented.
  • Provide support and assistance to medication management systems.
  • Ensure that medications are dispensed in a safe and effective manner, keep medication cart orderly, and all medications appropriately packaged and stored.
  • Monitor resident health concerns including weight and nutrition, skin integrity, mobility, fall management, and pain management.
Requirements:
  • Current licensure as a Practical Nurse (LPN) or Registered Nurse (RN) in the State of New Jersey.
  • 1+ year preferred but not required.
