Job description

RVNAhealth is currently seeking a Per Diem Registered Nurse (RN) or Licensed Practical Nurse (LPN) for 12-hour night and day shifts. These shifts may be long or short term. Will include hospice cases that require around the clock medication administered along with attention to ADL's.


Duties:
  1. Provides direct patient care to patients of all ages.
  2. Identifies and documents patient and family needs through comprehensive assessment.
  3. Performs evaluation of patients' progress, promptly acts on any changes made to patient's plan of treatment or condition.
  4. Updates and maintains the nursing care plan.
  5. Appropriately utilizes and maintains all nursing equipment and supplies needed for patient care. Interprets findings of tests to physician, patient, or family as appropriate.
  6. Counsels patients and families on the basic principles of wellness including information on related fields such as nutrition, safety, education, mental health, dental care, and overall health and wellness aspects.
  7. As needed, covers evening, weekend and holiday time for the agency.
  8. Assesses patients at risk for existing or potential situations of neglect and initiates referral to appropriate community agencies.
  9. Ability to maintain Universal Precautions, infection control and proper bag technique.
  10. Knowledgeable and compliant with all agency and regulatory procedures.
  11. Completes quality documentation within required agency and/or regulatory timeframes.
  12. Ability to establish respectful, positive working relationships with other employees and volunteers in the delivery of patient services.

Qualifications:
  1. Graduation from an accredited school of nursing.
  2. Currently licensed to practice as a Registered Nurse or Licensed Practical Nurse in the State of Connecticut.
  3. A minimum of one year of nursing experience, preferably in hospital setting. Home Health Care experience preferred. Hospice experience preferred.
  4. Is computer literate, enters information into computer in a timely and accurate manner. Uses electronic means of communication effectively within agency policies and practices.
