Job Overview

We are seeking a dedicated and compassionate Nurse to join our healthcare team. The ideal candidate will provide high-quality resident care, support the medical staff, and ensure residents receive the best treatment in settings including long term care, sub-acute care, and memory care. This role requires strong clinical skills, excellent communication, and a commitment to resident-centered care.

Responsibilities

1. Assess resident health problems and needs, develop and implement nursing care plans.
2. Administer medications and treatments as prescribed by physicians.
3. Monitor resident progress and respond to changes in condition.
4. Collaborate with multidisciplinary teams to provide comprehensive care.
5. Educate residents and families regarding health conditions, treatment plans, and discharge.
6. Maintain accurate resident records and documentation in compliance with regulations, including HIPAA.
7. Participate in case management activities to ensure continuity of care.
8. Provide support in specialized areas such as PT, OT, SUD, Pain Management, etc.
