Job summary

GreenTree HCM is seeking a PPS/ MDS Coordinator Assistant to support the Director of Nursing. The role focuses on coordinating MDS completion, RAI processes, and care plan development to maximize resident reimbursement while maintaining high-quality care.

You will chair care plan meetings and ensure timely, compliant documentation across the facility. The position requires RN/LPN licensure, 3 years in a nursing home, and strong knowledge of RAI/PPS with leadership abilities.

Qualifications

• 3 years of experience in a nursing home setting with rehabilitative focus.
• Understanding of RAI and PPS processes with leadership capacity.
• Knowledge of long-term care regulations and HIPAA considerations.

Responsibilities

• Coordinate the completion of MDS and related processes for care planning and reimbursement.
• Chairs care plan meetings and ensures accuracy of case mix data.
• Schedule and coordinate RAI forms and PPS tasks across interdisciplinary staff.
• Ensure documentation supports RAI and PPS data and is transmitted to the state.
