Scope of Work

Responsible for the accurate completion of the Minimum Data Set (MDS) and Care Area Assessments (CAAs), collaborating with Staff Nurses, Nurse Aides, and the interdisciplinary team on the assessment process and development of resident MDS/Care Plans. The position will monitor Medicare and Medicaid requirements.

About the Facility

Beaumont Commons – Farmington Hills

Our facility has 3 floors and 9 units (3 units per floor). We provide care for long-term care residents and post-acute rehab patients. The first floor has 59 beds focusing on sub-acute rehab. The second floor has 59 beds focusing on long-term care and sub-acute rehab. The third floor is a secured dementia unit. Nurse-patient ratios vary depending on the shift and the population served. We care for residents with dementia, long-term care needs, and sub-acute rehab. With 179 beds, we accept various patient specialties, including general rehab, neuro rehab, cardiac, respiratory, and cancer. We also offer various programs for residents to participate in multiple activities. Our goal is to create a Fullness of Life through the joy of relationships, the art of caring, and the spirit of living.

Qualifications

  • Required: A level of knowledge normally acquired through the successful completion of a Licensed Practical Nurse Program.
  • Required: 3 years of relevant experience as a Licensed Practical Nurse in a long-term care setting.
  • License: Licensed Practical Nurse (LPN) - STATE_MI upon hire.
