Job summary

Smoky Mountain Health & Rehabilitation Center in Pigeon Forge, TN is seeking an experienced RN or LPN MDS Coordinator to join our team. You will coordinate the MDS/RAI process, ensure accuracy and timely completion of MDS, CAAs, and interdisciplinary care plans, and work with the nursing leadership to support compliant reimbursement.

Qualifications

• Current, active Tennessee RN or LPN license.
• Nursing degree or diploma from an accredited program.
• Current CPR certification.
• Minimum of 2 years Long-Term Care MDS Coordinator experience.
• Strong MDS/RAI knowledge and care planning experience.
• RAC-CT certification preferred.
• Experience with Medicare, Medicaid, reimbursement, and Case-Mix preferred.

Responsibilities

• Coordinate and oversee completion of MDS assessments within required timeframes.
• Monitor the interdisciplinary team through the RAI/MDS process.
• Review MDS documentation for accuracy, completeness and regulatory compliance.
• Oversee submission and transmission of MDS assessments and validate submissions.
• Coordinate and manage the resident care planning process.
• Schedule and facilitate interdisciplinary care plan conferences.
• Ensure care plans reflect residents' current needs and goals.
• Facilitate monthly QM meetings and monitor quality measures for improvement.
• Maintain compliance with federal/state/local regulations.
• Collaborate with nursing, therapy, dietary, social services and others.
• Support accurate reimbursement through proper assessment, documentation and coding.
