Job summary

Omega Senior Living Llc is seeking a Health and Wellness Coordinator to work under the Health and Wellness Director, coordinating health services and maintaining care plans for residents. You will supervise assistants, verify orders, and advocate for residents in collaboration with the Executive Director.

The role requires nursing licensure (LPN/LVN or RN per state), teamwork, and a proactive approach to infection control and documentation. Night availability is expected.

Qualifications

• Must be a licensed LPN/LVN or RN per state guidelines.
• Must demonstrate knowledge to provide age-appropriate care.
• Must be a supportive team member and contribute to teamwork.

Responsibilities

• Adhere to dignity, privacy, independence, choice, and individuality for Residents.
• Coordinate ancillary health care and health education services with the Executive Director.
• Verify physician orders and move-in/move-out information; take appropriate action.
• Identify Resident problems and ensure follow-up to resolution.
• Be available at night as needed and act as Resident advocate.
• Promote infection control procedures and track quality trends.

Education

Licensed LPN/LVN or RN (as required by state guidelines)
