Job summary

Hireology is hiring a Wellness Nurse (RN/LPN) to coordinate resident care in a senior living community. The role focuses on compassionate, accurate care and leadership of Resident Care Aides (RCAs). You will liaise with families, physicians, and care partners while ensuring regulatory compliance.

Your responsibilities include assessing health concerns, documenting changes, and maintaining high standards of care in a team-oriented environment.

Qualifications

• Current, valid RN or LPN license in the state of employment.
• Knowledge of aging and disability-related care.
• Strong assessment, communication, and documentation skills.
• High attention to detail and commitment to accuracy.
• Patient, compassionate, and service-oriented mindset.
• Ability to work collaboratively and lead with integrity.

Responsibilities

• Provide resident-centered care with exceptional quality.
• Assess health concerns and coordinate care with providers.
• Observe, document, and communicate changes in resident condition clearly.
• Provide first aid and clinical support as needed.
• Mentor and support RCAs in daily care delivery.
• Collaborate to ensure seamless, high-quality care.
• Serve as liaison between residents, families, physicians and agencies.
• Maintain detailed, accurate documentation in compliance with regulations.
