Job summary

Paycom - ATS is seeking a licensed LPN/LVN or RN to support resident care coordination, ensure accurate documentation, and advocate for residents under the supervision of the Health & Wellness Director. The role emphasizes infection control, teamwork, and independent decision-making in a skilled nursing environment.

Qualifications include an active LPN/LVN or RN license, state-specific requirements, and preferred experience in AL or SNUs.

Qualifications

• Current license as a LPN/LVN or RN (as required by state guidelines).
• Must be licensed and have any required examinations under state law.
• Prefer prior experience in AL or SNU.
• Prefer prior experience as a LPN/LVN or RN for at least 1 year.
• Must demonstrate knowledge to provide age-appropriate care.

Responsibilities

• Adhere to and convey a philosophy of supporting dignity, privacy, independence, choice, and individuality for Residents.
• Assist in coordinating ancillary health care and health education services for Residents.
• Ensure documentation including service and medication delivery is accurate and compliant with regulations.
• Act as Resident advocate in medical/social situations under direction of the HWD.
• Promote infection control procedures among Associates.
• Track trends for Quality Management Review.
• Assist in verifying physician orders, move-in/move-out information, and take appropriate follow-up actions.
• Help identify Resident problems and ensure proper follow-up to resolution.
• Nighttime HW Coordinator must be immediately available and awake.
