Tradewater Health & Rehab, a Concord Health Systems facility in Western Kentucky, is seeking dependable LPNs and RNs for multiple shifts. This locally owned nursing home group offers flexible scheduling across day, night, and weekend shifts to provide high-quality care in a skilled nursing and long-term care setting.

Formación

• Current LPN or RN license in Kentucky.
• Experience in long-term care, skilled nursing, or nursing home preferred.
• Strong clinical, organizational, and communication skills.
• Reliable attendance and flexibility to work assigned shifts.

Responsabilidades

• Administer medications and treatments per physician orders and policy.
• Assess (RN) or monitor (LPN) resident conditions and report changes.
• Provide direct nursing care in a skilled nursing and long-term care setting.
• Document care and treatments in EMAR/EHR system accurately.
• Collaborate with CNAs and interdisciplinary team to deliver care.
• Assist with care planning and follow established plans.
• Maintain compliance with state and federal regulations.
• Contribute to a respectful, resident-centered environment.

Shift: Day Shift | Night Shift | Weekend Options Available

Status: Full-Time and Part-Time Positions Available /Baylor weekends

Position Summary

We are seeking dependable and compassionate Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to join our team in a locally owned and operated nursing home group serving Western Kentucky.

This position offers flexible scheduling across multiple shifts, allowing nurses to find a schedule that fits their needs while providing high-quality care in a skilled nursing and long-term care environment.

Essential Duties

• Administer medications and treatments in accordance with physician orders and facility policy
• Assess (RN) or monitor (LPN) resident conditions and report changes appropriately
• Provide direct nursing care in a skilled nursing and long-term care setting
• Accurately document care and treatments in the medical record system (EMAR/EHR)
• Collaborate with CNAs and interdisciplinary team members to deliver quality care
• Assist with care planning and follow established care plans
• Maintain compliance with state and federal regulations
• Contribute to a respectful, resident-centered environment

Qualifications

• Current LPN or RN license in the state of Kentucky
