Job summary

The Health Center at Bloomingdale seeks an LPN or RN for the 7-3 shift with a competitive weekend differential program. This role covers all shifts and operates within our five-star skilled nursing facility.

You will participate in resident assessments, care conferences with families, monitor health changes, assist with medication management and safe dispensing, and collaborate with residents, families, and a multidisciplinary team to maintain high-quality, personalized care.

Qualifications

• Current licensure as a Practical Nurse (LPN) or Registered Nurse (RN) in New Jersey.

Responsibilities

• Assist with resident assessments and care plan implementation; participate in care conferences with residents and families.
• Communicate effectively with residents, families, staff, physicians, outside providers and the public.
• Monitor changes in resident condition; document and implement effective interventions.
• Provide support for medication management systems.
• Ensure that medications are dispensed safely, keep medication cart orderly, and medications properly stored.
• Monitor health concerns including weight, nutrition, skin integrity, mobility, falls, and pain.

Responsibilities:

• Assist with resident assessment and implementation of care needs and will participate in resident and family care conference
• Must be able to effectively communicate with residents, families, staff, physicians, outside providers and the public
• Ensures that changes in resident condition are evaluated, monitored, documented and effective interventions are implemented
• Provide support and assistance to medication management systems
• Ensure that medications are dispensed in a safe and effective manner, keep medication cart orderly, and all medications appropriately packaged and stored
• Monitor resident health concerns including weight and nutrition, skin integrity, mobility, fall management and pain management
