Job summary

The Health Center at Bloomingdale, a five-star skilled nursing facility, is seeking an LPN or RN for per diem weekend shifts across all hours. You will participate in resident and family conferences, assess care needs, and collaborate with a multidisciplinary team to optimize care plans.

Responsibilities include ongoing condition monitoring, medication support, safe dispensing, and attention to nutrition, skin integrity, mobility and pain management. NJ licensure is required.

Qualifications

• Current licensure as a Practical Nurse (LPN) or Registered Nurse (RN) in New Jersey.

Responsibilities

• Assist with resident assessment and care needs and participate in resident and family care conferences.
• Communicate effectively with residents, families, staff, physicians, and external providers.
• Monitor changes in resident condition and ensure interventions are implemented.
• Support medication management processes and ensure safe dispensing of medications.
• Keep medication cart orderly and ensure proper storage and packaging of medications.
• Monitor nutrition, skin integrity, mobility, falls, and pain management.

Responsibilities:

• Assist with resident assessment and implementation of care needs and will participate in resident and family care conference
• Must be able to effectively communicate with residents, families, staff, physicians, outside providers and the public
• Ensures that changes in resident condition are evaluated, monitored, documented and effective interventions are implemented
• Provide support and assistance to medication management systems
• Ensure that medications are dispensed in a safe and effective manner, keep medication cart orderly, and all medications appropriately packaged and stored
• Monitor resident health concerns including weight and nutrition, skin integrity, mobility, fall management and pain management

Requirements:
