Our Stillwater team is currently hiring. Buurtzorg: Neighborhood Nursing is an innovative, community-based, integrated care model. The concept developed in the Netherlands, where professional nurses work in autonomous teams (up to 12) and take responsibility for all care needs of their clients, both professional and personal, from intake to discharge.

Built on the belief that nurse professionals have a more fulfilling work experience through self‑management and effective collaboration, middle or top‑down management becomes unnecessary. Bringing highly skilled nurses into our neighborhoods results in high quality of care delivered, improved health outcomes through prevention and early intervention. Our approach is holistic, and begins with the relationship between client and nurse.

Job Description

Deliver highest quality of care for clients in their homes, with their goals and preferences as the starting point. Ensure short lines of communication between physician, professionals and caregivers. As a health coach, work toward stability and autonomy. Connect clients with existing supports in the community. Offer flexible hours, competitive wages, and share on‑call responsibility between colleagues.

Qualifications

Licensure in MN and WI. Desire to collaborate with colleagues in a supportive, positive, solution–oriented environment using self‑management and solution–oriented techniques to operate as a small, local team. Ability to function independently, providing care to the full professional scope of licensure.
