West State Street is seeking an experienced Registered Nurse (RN) to deliver high-quality patient care in a long-term care setting. Under the supervision of the Nurse Supervisor or Unit Manager, the RN will assess patients, develop care plans, and coordinate with LPNs and CNAs to achieve optimal outcomes.

The role emphasizes compassionate service, HIPAA compliance, and accurate PCC EHR documentation, with opportunities for Continuing Education and specialty certification support.

Qualificações

• The RN must be a graduate of an accredited School of Nursing and hold an active RN license.
• Current BLS/CPR certification is required.
• Experience in long-term care, post-acute, rehabilitative, or geriatric nursing is preferred.
• Proficient in English reading, writing and speaking for documentation and safety.
• Must meet health screenings and TST verification as mandated.

Responsabilidades

• Deliver comprehensive nursing care under supervision to achieve positive clinical outcomes.
• Perform assessments, nursing diagnoses, care planning, and interdisciplinary collaboration.
• Supervise and delegate tasks to LPNs and CNAs within state practice acts.
• Maintain real-time documentation using PCC EHR; ensure HIPAA compliance.
• Support patient/family satisfaction and discharge planning.
