Job summary

UW Health in Beaver Dam, Wisconsin is seeking a Registered Nurse for Long Term Care at Hillside Manor. The role focuses on resident-centered care, care coordination, and collaboration with a multidisciplinary team in a Medicare/Medicaid-certified facility.

The schedule is 90% FTE with 12-hour days, every other weekend and holiday rotations. On-site duties at Hillside Manor include supporting transitions of care and maintaining high-quality nursing care.

Qualifications

• Bachelor's Degree in Nursing preferred.
• No work experience required.
• Registered nurse licensure in the state where employed or licensure in a state in the nurses compact at hire

Responsibilities

• Provide compassionate, resident-centered care at Hillside Manor Nursing Facility, a Medicare/Medicaid-certified skilled nursing facility.
• Coordinate and manage individualized nursing care by assessing resident needs and developing plans of care.
• Administer treatments and collaborate with residents, families, and the interdisciplinary team.
• Assist residents through recovery and transitions of care with physicians and therapy services.
• Educate residents and families and participate in care conferences and shared decision-making.
• Support a culture of quality, safety, and teamwork under the Wisconsin Nurse Practice Act.
