• Provide compassionate, resident-centered care at Hillside Manor Nursing Facility.
• Coordinate and manage individualized nursing care by assessing needs and plans.
• Collaborate with physicians, therapy services, and other professionals for safe transitions.
• Educate residents and families, participate in care conferences and shared decision-making.
• Support quality and safety through adherence to Wisconsin Nurse Practice Act and evidence-based practices.
