The ideal candidate must be a graduate of an approved nursing, therapy or social work program with at least two years of field experience. You will work closely with healthcare professionals and provide quality care based on evidence-based guidelines.

Qualifications

• Must be licensed in the state where they currently practice.
• Two years of demonstrated field experience required.
• Basic demonstrated technology skills are a must.

• Develop transition plans that are patient-centered and promote quality care.
• Establish strategic relationships with health systems and hospitals.
