Responsibilities

• Monitor, assess, and evaluate the patient's condition, coordinating with the care team to adjust the care plan as needed.
• Deliver safe, high-quality care and document changes in condition, ensuring compliance with policies and procedures.
• Advocate for patients and families, communicating clearly with the care team and providing education as appropriate.
• Participate in performance improvement initiatives and uphold confidentiality, safety, and professional standards.
