The US Department of Veterans Affairs is seeking a Home Telehealth Care Coordinator (RN). The HTCC coordinates care for a defined high-resource patient population with the aim of preventing disease complications and maximizing functional ability.

The registered nurse will demonstrate leadership, clinical expertise, and creative approaches to manage complex patient care, ensuring goal-oriented, holistic services that improve quality of life for Veterans.

Qualifications

• Must be a graduate of an approved school of professional nursing.
• Current, full, active, and unrestricted nursing license in the United States.
• Foreign education may require degree equivalency validation by VA or equivalent body.
