Hybrid Schedule

The RN Lead Care Manager will engage, support, and provide education to ECM members with complex medical and psychosocial conditions.

Duties/Responsibilities:

• Conduct Comprehensive Health Assessments, Medication Reconciliation, and develop shared care plans with ECM members, revising regularly.
• Ensure smooth care transitions by coordinating with hospitals and care transition teams.
• Supervise Community Health Workers to provide education and support for members' community engagement and appointment accompaniment.
• Collaborate with Care Coordinators to identify appropriate community support services.
• Manage claims filing, billing updates, and documentation templates to ensure accuracy and effectiveness.
• Work with Quality Project staff to develop processes and resources for expanding ECM programs.
• Participate in quality, utilization review, and provider meetings.

Qualifications:

• Unencumbered California RN license; BSN preferred.
• BLS Certification.
• Minimum three years' experience in case management, public health, or related fields.
• Leadership qualities and proficient communication skills, including Motivational Interviewing and conflict resolution.
• Commitment to professional development and quality improvement.

Knowledge, Skills, and Abilities:

• Excellent interpersonal skills and cultural competence.
• Strong documentation skills and proficiency with Microsoft Office and Electronic Health Records.
• Bilingual English/Spanish preferred.
• Flexibility to work evenings and travel as needed.
• Reliable transportation and ability to work independently and as part of a team.

Physical and Environmental Requirements:

• Sitting for extended periods, light lifting, fine motor skills, and full-body mobility.

Work Schedule:

• Typically Monday through Friday, 8:00 or 9:00 a.m. to 5:00 or 6:00 p.m., with occasional Saturday work and travel to other locations.
