Job summary

Qualifications

• Associate Degree in Nursing.
• State of NM nursing license or compact license.
• BLS certification required at time of hire.
• ACLS certification.
• PALS certification.

Responsibilities

• Oversees delivery of patient care and delegates assignments based on acuity.
• Maintains clinical competence, evaluates outcomes and consults specialists as required.
• Coordinates care with patient, family, physicians and community resources.
• Supports patients and families with information and care.
• Explains rights, informed consent and directives during admission.
• Maintains knowledge of current nursing standards and hospital policies.
• Maintains proficiency with equipment used in practice.
• Communicates effectively with the nursing team and disciplines.
• Performs on-call duties and triage by phone and in person.
• Directs education for top ICD9 codes/diagnoses related to common conditions.
• Performs other duties as required.

Responsibilities:

• Acts as clinical resource by overseeing the delivery of patient care, directs and delegates patient care assignments to home health aides and nursing staff based on acuteness of patient care needs and staff capability and anticipates future needs
• Maintains clinically competent in providing direct patient care, evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure optimal patient care.
• Coordinates patient care through collaboration with the patient, family, all other disciplines, primary and secondary physicians and community resources/programs.
• Demonstrates knowledge philosophy and provides support to patients and their families.
• Ensures that patients understand their rights and responsibilities, advanced directives and the meaning of informed consent during the admission process.
• Maintains professional responsibility for knowledge of current nursing research, practice standards, guidelines and protocols, and hospital policies, procedures, and programs to improve patient care quality.
• Maintains a proficiency in the use of all equipment used in his/her practice.
• Effectively communicates and assists other members of nursing team and/or other disciplines. Actively participates in problem solving and conflict resolution to maintain team cohesiveness.
• Performs on-call duties effectively with the ability to triage patients over the phone and visit when needed.
• Directs own educational needs for the top 10 ICD9 codes and/or diagnoses for diabetes, wounds/post-surgical, arthritis, newborns, Coumadin dependency, malnutrition/ nutrition needs, medication instruction, pain and symptom control
• Performs other functions as required.

• Associate Degree in Nursing

• BLS certification REQUIRED at time of hire
