Job summary

Cincinnati Children’s is seeking an RN Kidney Transplant Coordinator to manage the full continuum of care for kidney transplant patients and living donors. This role supports the multidisciplinary transplant team by coordinating medical, surgical, and psychosocial care with emphasis on adherence and seamless transitions across perioperative, post-transplant, and referral phases.

Qualifications

• Bachelor’s degree in Nursing is required.
• Active RN license in the state of Ohio (or ability to obtain).
• Minimum 5 years of clinical nursing experience, preferably in transplant, nephrology, or critical care.

Responsibilities

• Coordinate outpatient care to optimize recipient recovery and quality of life.
• Assess and support patient adherence to treatment and medication regimens.
• Identify complications and collaborate with providers to determine interventions.
• Arrange diagnostic testing, hospital admissions, and emergent evaluations.
• Maintain detailed electronic health records and serve as a liaison between patients, families, and the transplant team.
