Qualifications

• Must possess a current Arizona RN license.
• BLS certification is required.
• Associate Degree in Nursing (ADN) or Diploma nurse; BSN is preferred.
• Two years behavioral health experience required.
• Prior Intake experience is a plus.

Responsibilities

• Assesses patient needs and coordinates intake under nurse guidance.
• Formulates a plan of care utilizing assessment data and input from patient, family and health team.
• Implements the plan of care through coordination and supervision of the health care team.
• Documents assessment, planning, implementation and evaluation in the patient record.
• Assists with determining patient benefits/eligibility and throughput of patients.
