Americare seeks an experienced RN Intake Hospital Liaison to coordinate the intake process for patients referred to home care services. You will act as the main link among hospitals, referral sources, patients, families, and internal teams to ensure a seamless transition from hospital to home.

Responsibilities include reviewing referrals, coordinating home care services, facilitating timely admissions, and providing exceptional customer service with professionalism, compassion, and efficiency.

Qualifications

• Current NYS RN License.
• Allscripts is a MUST.
• CHHA experience is a MUST.
• Computer literacy; experience with Home Care Homebase is a MUST.
• Excellent communication skills.
• Highly organized; strong attention to detail.
• Experience multitasking and working in a high pressure environment.

Job Description:

The RN Intake Hospital Liaison is a Registered Professional Nurse responsible for coordinating the intake process for patients referred to home care services. Serving as the primary liaison between hospitals, referral sources, patients, families, and internal teams, this role ensures a seamless transition from hospital to home. Responsibilities include reviewing referrals, coordinating appropriate home care services, facilitating timely admissions, and providing exceptional customer service by addressing patient inquiries and concerns with professionalism, compassion, and efficiency.
