Job summary

Mercy is seeking a Home Health RN – Intake to manage Hospice, Home Health and Home Infusion referrals across acute care, clinics and nursing facilities. You will ensure timely discharges and coordinate with discharge planners, social workers and physicians.

The role requires RN licensure in the applicable state, with a minimum of three years nursing experience and Home Health exposure preferred; hours are 8am-4:30pm with comprehensive Mercy benefits.

Qualifications

• RN license in applicable state required.
• Minimum three years of nursing experience required.
• Experience in Home Health preferred.
• Inpatient acute and community settings experience preferred.
