Job summary

Henry Ford Health is seeking a Registered Nurse Intake Coordinator for HF at Home. This full-time role focuses on gathering accurate patient demographics and clinical information before discharge, assessing eligibility for at-home services, and building strong relationships with referral sources and the Henry Ford at Home team.

The position requires an accredited RN program graduate with Michigan RN licensure and at least one year of clinical nursing experience; IV experience is preferred.

Qualifications

• Graduation from an accredited Registered Nurse program.
• Current licensure by the State of Michigan as a registered nurse.
• Minimum of one year clinical nursing experience; IV administration preferred.
• Strong analytical skills to solve problems and develop procedures.
• Previous sales, health care marketing or similar experience preferred.
• Ability to communicate effectively and maintain relationships with the medical team.
• Knowledge of regulatory/reimbursement requirements for Henry Ford at Home products and services and coverage-approval channels preferred.
• Proficient computer use to electronically document patient care.
• Must have a valid driver's license and reliable transportation.
• Ability to lift, carry, transport and move clients as needed.

Responsibilities

• Gather demographic and clinical patient information prior to discharge and start of care.
• Assess patient for appropriateness of Henry Ford at Home services.
• Educate and strengthen relationships with referral sources and Henry Ford at Home providers.
• Coordinate with hospital/facility case managers for smooth transitions.
• Provide home infusion visits in patient homes and manage patient caseloads.
