Job summary

Henry Ford Health in Michigan seeks a Home Health Registered Nurse to provide direct patient care in patients' homes across Detroit, Dearborn, Melvindale, and Allen Park.

Under supervision of the nursing supervisor, you will assess needs for allied health services, coordinate with rehab and social work, and ensure an integrated home health delivery system with mileage reimbursement.

Qualifications

• Successful completion of an RN program accredited by the NLN.
• Current licensure by the State of Michigan as a registered nurse
• Minimum of nine (9) months of clinical nursing experience required.
• Ability to effectively communicate with patients, families and hospital personnel.
• Strong interpersonal and customer service skills for diverse backgrounds.
• Must possess a current State of Michigan driver's license and a satisfactory driving record.
• Must have reliable transportation and a telephone.
• Must be able to lift, carry, transport and move clients, if needed.

Responsibilities

• Provide direct patient care in the home under supervision.
• Assess need for other allied health care services to ensure integrated home health care.
• Coordinate with home health aides, physical therapy, occupational therapy, dietetics, SLP, and social work.
• Work under the IRS Mileage Reimbursement rate.

GENERAL SUMMARY:

Assesses need for other allied health care services such as home health aide services, physical therapy, occupational therapy, dietetics, speech-language pathology, and social work, to ensure an integrated home health care delivery system. Over the IRS Mileage Reimbursement rate.

EDUCATION/EXPERIENCE:

• Successful completion of a registered nursing program accredited by the National League of Nursing.
• Current licensure by the State of Michigan as a registered nurse
• Minimum of nine (9) months of clinical nursing experience required.
• Ability to effectively communicate with patients, families and hospital personnel.
• Strong interpersonal and customer service skills required in order to interact effectively with individuals from various cultural and socio-economic backgrounds.
• Must possess a current State of Michigan driver's license and a satisfactory driving record.
• Must have reliable transportation and a telephone.
• Must be able to lift, carry, transport and move clients, if needed.

CERTIFICATIONS/LICENSURES REQUIRED: Licensure as a Registered Nurse in the State of Michigan.
