Job summary

UT Southwestern Medical Center in Dallas, TX seeks a Nursing Intake and Care Coordination professional. You will manage patient referrals, assess admission appropriateness, and coordinate with hospital departments to ensure smooth intake and care transitions.

Requires NLN-approved nursing education, Texas license or NLC, and BLS. 3+ years in medical intake/care coordination preferred. Full-time, day shift in the Heart Lung Vasc Svc Admin unit.

Qualifications

• Graduate of NLN approved nursing program.
• Experience in medical intake and/or care coordination is required.
• Licensure in Texas or NLC licensure is required; BLS certification is required.

Responsibilities

• Receive and respond to inquiries and patient referrals, establishing initial relationships.
• Determine admission appropriateness based on criteria and services.
• Coordinate with departments and physicians to facilitate admission.
• Assess financial reimbursement and obtain pre-certifications for interventions.
• Communicate service overview to patients and payers; ensure availability of needed resources.
• Document intake data and referral sources; train staff on intake processes.
