Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• 1–2 years hospice/palliative or acute care experience.

Responsibilities

• Provide skilled nursing care per Plan of Care and IDT/IDG guidelines.
• Coordinate patient care across disciplines and communicate with providers.
• Educate patients and families about hospice philosophy and care options.
• Supervise CNAs and participate in staff training and orientation.
