Job summary

Pathways Hospice in Fort Collins, CO is seeking a Registered Nurse to join the Nights team in the Inpatient Care Center. This role follows a Thursday/Friday/Saturday 7p-7a schedule, providing expert nursing care within a supportive care environment.

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• 1-2 years experience in hospice and/or palliative care, medical/surgical, public health, oncology, acute care or home health.

Responsibilities

• Provide skilled nursing coordination of care per the Plan of Care.
• Collaborate across disciplines in IDT/IDG meetings.
• Educate patients and families on hospice philosophy and care options.
