Qualifications

• Ability to interrelate with physicians, nurses, support staff, and patients.

Responsibilities

• Coordinate post-discharge care to ensure timely discharge for routine and complex patients.
• Perform thorough, timely patient assessments and document care needs and benefits.
• Develop mutually agreeable care plans with patients and families.
• Facilitate safe transfers and optimize resource use across care settings.
• Ensure regulatory and compliance standards are met with the care team.
