Job summary

Aetna in Salem, USA is seeking a Registered Nurse to support discharge planning, coordinate care with case managers and physicians, and collaborate with home health organizations.

You will manage therapy offerings, training for patients and caregivers, and contribute to quality improvement initiatives across assigned hospitals. A current RN license and IV infusion experience are required, along with CPR certification and a valid driver's license.

Qualifications

• Active RN license in the state of employment.
• At least one year IV infusion experience.
• Current CPR certification.
• Driver's license and automobile insurance where required.

Responsibilities

• Coordinate with case managers, physicians, referral sources, and home health organizations to facilitate patient discharge and care coordination.
• Conduct infusion setup and patient/caregiver training on therapy offerings.
• Assess management approach based on physician orders.
• Suggest formulary modifications and guide feeding/vitamin recommendations as needed.
• Identify opportunities to improve patient care and participate in quality/performance projects.
