Job summary

You will lead and mentor LPNs/CNAs, ensure smooth care transitions, and maintain real-time documentation in PCC. A BSN/ADN, active RN license, BLS, and experience in long-term care are preferred, with opportunities for growth within Genesis.

Qualifications

• Education: BSN or ADN from an accredited program.
• Licensure: Active RN license.
• Certifications: BLS/CPR current.
• Clinical Experience: Long-term care/post-acute/geriatric preferred.
• Language: Proficient English for documentation and communication.
• Health: Meets state health requirements including TST.

Responsibilities

• Perform comprehensive clinical assessments and develop care plans.
• Administer medications and treatments per physician orders.
• Lead and delegate to LPNs and CNAs in the care team.
• Collaborate with physicians and interdisciplinary staff for patient care and discharge planning.
