Job summary

Senior Community Care of Kentucky PACE in Louisville, KY is seeking a Registered Nurse to provide general and skilled nursing care to participants in their homes and community settings, implement the plan of care, and educate participants and families.

This full-time role includes coordinating with the Interdisciplinary Team, performing initial assessments, and ensuring care continuity for the frail and elderly population.

Qualifications

• Current licensure in Kentucky as a Registered Nurse.
• Must have a valid driver’s license and transportation.
• Medical clearance for communicable diseases and up-to-date immunizations.
• Current CPR Certification.
• Minimum 1 year experience with frail or elderly population.

Responsibilities

• Provide general and skilled nursing care per plan of care.
• Coordinate care with Interdisciplinary Team to support safe living in the community.
• Conduct initial nursing assessment and semi-annual re‑assessments.
• Perform nursing care and administer physician orders in coordination with the team.
• Participate in on-call rotation for after-hours and weekend care.
• Maintain participant records and documentation per policy.
