Job summary

Elwyn is seeking a Registered Nurse to join the Interdisciplinary Care Team. You will perform health assessments, monitor medical and psychological health, and collaborate with physicians and therapists to support clients with developmental disabilities and behavioral health needs.

You will document care in MAR/eMAR, administer medications, and ensure patient privacy while delivering compassionate, quality care in a fast-paced environment.

Qualifications

• Licensed RN in Pennsylvania with current licensure.
• Minimum 1 year of general nursing or medical-surgical experience.
• Experience using an electronic medical record system.

Responsibilities

• Perform comprehensive nursing assessments on admission and as health needs change.
• Complete quarterly nursing assessments and Braden scale/fall risk reviews.
• Prepare discharge summaries with medications and follow-up care.
• Develop healthcare plans for 24/7 nursing care with IDT input.
• Document in MAR/eMAR and ensure patient privacy during administration.
• Coordinate with physicians and therapists to implement care plans.
• Monitor vital signs, weight, and BP monthly or as ordered.
• Provide patient and family education and updates to Charge Nurse.
