Job summary

Muir Home Health in the East Bay of California seeks a Skilled Nursing Facility Liaison to represent at local hospitals, educate referral sources and families about home health services, and drive high-quality referrals. You will leverage clinical knowledge, communication, and marketing skills to explain care options and coordinate plans with physicians.

The role requires a CA Registered Nurse License, 2+ years nursing experience (home health preferred) and 2+ years marketing experience, plus

Qualifications

• Valid CA Registered Nurse License.
• 2+ years nursing experience, home health setting preferred.
• 2+ years of marketing experience.
• Maintains CPR certification.

Responsibilities

• Educate the community about Muir Home Health services and build relationships with families, caregivers, medical professionals, and communities.
• Coordinate with physicians and referral sources regarding Home Health plans of care.
• Review clinical documentation to determine Home Health appropriateness.
• Review patient charts and understand pharmaceuticals, test results, therapy evaluations and pre-existing conditions.
• Communicate with patients, families and caregivers about Home Health services.
• Conduct thorough patient assessments to identify potential admissions.
• Develop educational and community relations plans with agency leadership.
• Maintain knowledge of local, state and federal home health regulations and care plan techniques.
