Qualifications

• Current RN license in state of practice.
• BSN within 4 years of hire or promotion; education agreement may take precedence.
• Inpatient hospital nursing experience and familiarity with hospital terminology.
• Understanding of disease management, including barriers to care.

Responsibilities

• Assess patients using screening criteria and perform comprehensive evaluations.
• Develop, adjust, and coordinate care plans addressing medical and social needs.
• Guide self-management using motivational interviewing and coaching.
• Educate and advocate for patient rights; support self-care.
• Collaborate with multidisciplinary teams to ensure timely, high-quality care.
• Lead process-improvement initiatives for care transitions and policy adherence.
• Provide Goals of Care education for informed decision-making.
