Job summary

Intermountain Health is seeking an RN Hospital Care Manager I to deliver comprehensive care management across inpatient and outpatient settings. You will develop patient-centered plans, collaborate with families, providers, and payers, and lead efforts to ensure seamless care transitions.

The role requires clinical nursing experience in hospital settings, BSN, and active RN licensure; case management certification is preferred.

Qualifications

• Current RN license in state of practice.
• BSN from an accredited institution; must obtain BSN within four years of hire/promotion if required.
• Demonstrated inpatient hospital nursing experience and familiarity with hospital terminology and processes.
• Understanding of disease management, barriers to care, and variation in treatment.

Responsibilities

• Assess patients using screening criteria and conduct comprehensive evaluations.
• Develop and coordinate care plans addressing medical and social needs.
• Guide self-management using motivational interviewing and coaching.
• Educate healthcare teams and patients; advocate for patient rights and support self-care.
• Collaborate with multidisciplinary teams to ensure timely, high-quality care.
• Lead initiatives to improve care transitions and policy adherence.
• Provide Goals of Care education for informed decision-making.
• Track key metrics, analyze trends, and ensure regulatory compliance.
