Qualifications

• Current RN license in state of practice.
• BSN required within four years or as per agreement.
• Experience in inpatient hospital settings and familiar with hospital terminology.
• Understanding of disease management and barriers to care.

Responsibilities

• Assess patients using screening criteria and conduct evaluations.
• Develop and coordinate care plans addressing medical and social needs.
• Guide self-management using motivational interviewing and coaching.
• Educate and advocate for patients; support self-care.
• Collaborate with multidisciplinary teams to ensure timely, high-quality care.
• Lead process improvements for care transitions and policy adherence.
• Provide goals-of-care education for informed decisions.
• Track metrics, analyze trends, and ensure regulatory compliance.
