Job summary

Pathways Healthcare is seeking a part-time Home Health Nurse to provide in-home care across Westwood and surrounding areas. The role supports preventive and rehabilitative nursing procedures on weekends, coordinating with physicians and a multidisciplinary team to deliver patient-centered care.

The ideal candidate has 2+ years of nursing experience, an active RN license, and proficiency with EMR systems. A valid driver’s license and reliable vehicle are required for travel across the Westwood

Qualifications

• 2+ years of nursing experience, ideally in geriatrics.
• Proficient with electronic medical records systems.
• Excellent bedside manner and communication skills.
• Active BLS CPR certification.
• Valid driver's license and own reliable vehicle.

Responsibilities

• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates and revises the plan of care as patient needs change.
• Responds to afterhours calls during weekends.
• Assesss patient needs through triage and in-home care when needed.
• Collaborates with physicians and the care team to maintain a strong plan of care for each patient.
• Responds to inquiries from patients, caregivers, and medical team.
• Consults patients and caregivers before, during and after treatment.

Position Details:

• Main Office location: Westwood, MA
• Service Area: Westwood, Quincy, Weymouth, Holbrook, Brockton, Attleboro, Taunton areas
• Job Type: Part-time
• Schedule: Saturday and Sunday 8:30 am – 8:00 pm

Responsibilities:

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assesss patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.

Qualifications:

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Benefits & Compensation:

• Pay Structure: Base Compensation ($600 per weekend) + Paid per visit ($50) and per admission ($100). Visits over 60 minutes are paid at $75. Training and meetings are paid hourly at $35/hr.
