Job summary

Pathways Healthcare is a Westwood-based hospice care provider seeking an On-Call Hospice Triage Nurse to support after-hours triage, care planning, and coordination with clinicians.

The role is part-time with weekend shifts (Sat-Sun 8:30 am–8:00 pm) serving Westwood and Boston areas. Requires 2+ years of nursing experience, a nursing degree, and BLS CPR; EMR proficiency and a valid driver’s license are preferred.

Qualifications

• 2+ years of nursing experience, preferably geriatrics
• Degree in Nursing from an accredited institution
• Proficient with electronic medical records
• Excellent bedside manner and communication skills
• Working knowledge of healthcare laws and regulations
• Maintain current BLS CPR certification
• Possess a valid driver's license and reliable vehicle

Responsibilities

• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and revises as patient needs change.
• Responds promptly to after-hours calls (weekends 8:30 AM–8:00 PM).
• Assesses patient needs through triage and provides in-home care when needed.
• Collaborates with physicians, NPs and interdisciplinary team on the Plan of Care.
• Responds to inquiries from patients, caregivers, and medical team.
• Consults patients and caregivers before, during and after treatment.

Position Details:

• Main Office location: Westwood, MA
• Service Area: Westwood & Boston areas
• Job Type: Part-time
• Schedule: Saturday and Sunday 8:30 am - 8:00 pm

Responsibilities:

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assesses patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.

Qualifications:

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• New nurses, feel free to apply! We have a great training program.

Benefits & Compensation:

• Pay Structure: Base Compensation ($600 per weekend) + Paid per visit ($50) and per admission ($100). Visits over 60 minutes are paid at $75. Training and meetings are paid hourly at $35/hr.
• Mileage Reimbursement ($0.36/mile)
• 401(k) Match.
