Job summary

Cleveland Clinic in Independence, OH is seeking an RN Hospice PRN to deliver compassionate home-based care in the Greater Cleveland Area. The role emphasizes comfort, prevention of readmissions, and flexible scheduling.

You will supervise LPNs and aides, complete admissions and ongoing assessments, manage pain, and coordinate multidisciplinary care with patients and families, all while maintaining thorough documentation. Per diem night shifts at Cleveland Clinic Fairview Hospital.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• One year of clinical RN experience in an acute medical/surgical setting; current RN licensure required; minimum one year in acute med/surg or home care.
• Current RN licensure, BLS certification, Ohio driver's license and auto insurance.

Responsibilities

• Provide direct nursing care per policy, procedures and standards.
• Implement and monitor patient care plans.
• Supervise LPNs, Home Care Aides, and families involved in patient care.
• Perform admission assessments and ongoing reassessments within required timeframes.
• Assess safety risks, abuse, and neglect.
• Coordinate multidisciplinary patient care across all disciplines.
• Assess and document patient pain and manage analgesia.
• Prioritize and document problems with patients, families, and health team.
• Maintain up-to-date documentation on patient condition and care effectiveness.
• Escalate changes in patient condition to the physician as appropriate.

About This Role

• Schedule: Per Diem, Night shift

What You'll Do

• Provide direct nursing care in accordance with established healthcare organization policies and procedures.
• Implement and monitor patient care plans.
• Provide supervision to LPNs, Home Care Aides, and families/caregivers involved in patient care.
• Perform accurate and complete admission assessments and reassessments within a specified time frame.
• Assess patients for safety risks, abuse, and neglect.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Assess and document patients' pain using appropriate pain scale and manage pain.
• Prioritize and document patient problems based on assessed needs in collaboration with patients, family, and healthcare team.
• Maintain complete and up-to-date documentation regarding patient condition and care effectiveness.
• Escalate changes in patient condition to the patient’s physician as appropriate.

Requirements

• Education: Graduate from an accredited school of Professional Nursing
• Experience: One year of clinical experience as an RN in an acute medical/surgical setting; Minimum one year current clinical experience as a registered nurse; Minimum one year experience in an acute medical-surgical or home care setting
• Licenses & Certifications: Current state licensure as a Registered Nurse (RN); Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross; Valid Driver’s License (VDL) in Ohio and proof of automobile insurance with $100,000/$300,000 coverage
