Qualifications

• Graduate of an accredited school of professional nursing; BSN preferred.
• Minimum one year clinical RN experience in an acute medical-surgical setting; infusion therapy experience preferred.
• Minimum of six months experience as a home care nurse for home care only.
• Current state RN licensure.
• Basic Life Support (BLS) certification through American Heart Association.
• Valid Ohio driver’s license upon hire; per policy 8‑175 thereafter.
• Proof of automobile insurance coverage, upon hire; per policy 8‑175 thereafter.

Responsibilities

• Provide direct nursing care per policies, procedures, and protocols.
• Perform admissions/reassessments within specified time frame.
• Assess safety risks, abuse, and neglect.
• Coordinate care as primary liaison for disciplines.
• Assess and document pain; manage patient pain.
• Prioritize problems with patient, family, and healthcare team.
• Maintain up-to-date documentation of patient condition and care effectiveness.
• Escalate changes in patient condition to physician as appropriate.
• Supervise LPNs, home care aides, and family/caregivers.
• Perform other duties as assigned.

About This Role

The RN Homecare PRN position involves providing direct nursing care in accordance with established policies and protocols, while implementing and monitoring patient care plans. This role is ideal for nurses who are skilled in coordinating care and supervising support staff in a home care setting.

What You'll Do

• Provide direct nursing care in accordance with established policies, procedures, and protocols.
• Perform accurate and complete admission assessments/reassessments within specified time frame.
• Assess patients for safety risks, abuse, and neglect.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Assess and document patients’ pain using appropriate pain scale and manage patients’ pain.
• Prioritize and document patient problems based on assessed needs/problems in collaboration with patient, family, and healthcare team.
• Maintain complete and up-to-date documentation regarding patient condition and effectiveness of care.
• Escalate changes in patient condition to the patient’s physician as appropriate.
• Supervise LPNs, home care aides, and family/caregivers involved in patient care.
• Perform other duties as assigned.

Requirements

• Education: Graduate of an accredited school of professional nursing. BSN preferred.
• Experience: Minimum one year clinical experience as a registered nurse in an acute medical-surgical setting; infusion therapy experience preferred.
• Experience: Minimum of six months experience as a home care nurse for home care only.
• Licenses & Certifications: Current state licensure as Registered Nurse (RN).
• Licenses & Certifications: Basic Life Support (BLS) through American Heart Association (AHA).
• Licenses & Certifications: Valid Ohio Driver’s license upon hire; per policy 8‑175 thereafter.
• Licenses & Certifications: Proof of automobile insurance coverage with $100,000/$300,000 coverage, upon hire; per policy 8‑175 thereafter.
