Job summary

JourneyCare Hospice is seeking a Hospice Transitional Care Navigator in the Oak Lawn area. This clinical-sales role focuses on expanding upstream hospice referrals from physicians and hospitals, acting as a consultant and educator to care teams, and driving appropriate hospice engagement.

The candidate will review documentation, build patient profiles, and partner with field sales to educate providers, ensuring timely and dignified end-of-life care.

Qualifications

• A clinically trained – experienced business development and sales professional that will focus on expanding the reach and impact of clinical hospice services.
• Must be self-driven with hunter-sales mentality and strong communication, resilience, and adaptability.
• Experience building upstream referral relationships with physicians, hospitals, and providers.

Responsibilities

• Build upstream-referral relationships and transition hospice referrals from physicians and other referral sources.
• Serve as hospice clinical consultant and educator to hospital and physician care teams.
• Review hospital records to identify decline patterns and educate on hospice appropriateness.
• Collaborate with field sales to schedule consultative meetings with external care teams.
• Identify trends and partnership opportunities for earlier hospice engagement.
• Maintain care team workflows for information, planning, and education.
