Job summary

Oncology Voice Network LLC in Englewood, NJ seeks a Hospice Inpatient Clinical Coordinator to develop hospital partnerships, assess referrals for hospice enrollment, and provide General Inpatient (GIP) level of care with specialized clinical skills.

You will lead an interdisciplinary team, design care plans, educate referrers and families, coordinate transitions, and maintain thorough documentation to ensure regulatory compliance.

Qualifications

• RN license required by state. Represents essential clinical capabilities in hospice coordination.
• BSN preferred for advanced practice and leadership.
• Proven ability to collaborate with hospital teams and discharge planners.

Responsibilities

• Develops and strengthens relationships with hospital partners.
• Assess patients referred for hospice enrollment and coordinate GIP level of care.
• Lead interdisciplinary team meetings and develop individualized care plans.
• Educate referrers, patients and families about hospice services.
• Coordinate transitions from inpatient to home hospice and maintain accurate documentation.
