Job summary

Metropolitan Jewish Health System, Inc. is seeking an experienced Hospice Inpatient Clinical Coordinator to strengthen hospital partnerships, assess referrals, and provide GIP level care with compassion. The role combines clinical nursing leadership with program development.

You will conduct comprehensive assessments, collaborate with IDT, educate referrers and families, coordinate with providers, and maintain thorough documentation to support quality end-of-life care.

Qualifications

• RN licensure (New York State) and CPR-BLS required.
• BSN preferred; graduate from an accredited School of Nursing.
• Experience in Medical-Surgical, Community Health, hospice or oncology nursing is preferred.
• Familiarity with Microsoft Office applications.
• Ability to work flexible hours including weekends.

Responsibilities

• Develops strong business continuity relationships with hospital partners.
• Assess patients referred for hospice enrollment and coordinate admission to hospice.
• Coordinate symptom management for patients awaiting discharge to home hospice.
• Serve as the RN member of the Inpatient Hospice IDT and coordinate care.
• Develop the hospice nursing plan of care and make service recommendations.
• Participate in IDT meetings and daily GIP Huddle to communicate patient status.
• Coordinate transition plans for patients no longer qualifying for inpatient care.
• Provide education about hospice care to staff, patients, and families.
• Maintain accurate and current hospice nursing assessments and interventions.
