Job summary

Metropolitan Jewish Health System is seeking an experienced Hospice Inpatient Clinical Coordinator in New York. The role focuses on developing hospital partnerships, assessing referrals, and providing GIP level care with an interdisciplinary team.

You will evaluate referrals, coordinate with physicians and case managers, and ensure compliant care plans and documentation. Excellent communication and critical-thinking skills are essential.

Qualifications

• Graduate from an accredited School of Nursing; BSN preferred.
• A minimum of two years’ experience in Medical-Surgical or Community Health; hospice/oncology preferred.
• New York State RN licensure; CPR-BLS.
• Valid NYS Driver's license preferred.
• Familiarity with Microsoft Office applications.
• Ability to work flexible hours, including weekends.
• Ability to coordinate resources and personnel in an interdisciplinary team.
• Ability to work non-judgmentally with diverse patients and families.
• Excellent communication, listening, and organizational skills.
• Ability to public speak and think critically.

Responsibilities

• Develops strong business continuity relationships to support growth goals.
• Responds to information requests from hospital staff and handles hospice enrollment benefits.
• Gathers clinical information for eligibility using COPs, LCDs and regulations.
• Coordinates symptom management for patients awaiting discharge to home hospice.
• RN member of IDT; coordinates care for General Inpatient patients.
• Develops nursing care plans and makes service recommendations.
• Participates in IDT meetings and daily GIP Huddle.
• Coordinates transition plans for patients no longer qualifying inpatient care.
• Oversees clinical care and educates hospital staff on hospice care.
• Maintains accurate documentation of assessments and interventions.
