Job summary

AuthoraCare Collective in Burlington, NC is seeking an RN Hospice Home Care Case Manager for a full-time schedule (40 hours/week) Monday–Friday, 8:00am–5:00pm. CPR/BLS is required and NC RN licensure active.

The role manages care plans for hospice patients, collaborates with the IDG, educates families, and provides compassionate support while maintaining documentation and on-call duties as scheduled. Must have own transportation.

Qualifications

• Must be licensed as an RN in North Carolina and in good standing.
• CPR certification from American Heart Association or American Red Cross BLS is required.
• Experience in hospice/home health or long-term care is a plus.
• Valid state driver's license and own transportation are required.

Responsibilities

• Manage the overall plan of care for a caseload of hospice patients.
• Assess patient/caregiver needs, establish nursing goals and interventions.
• Collaborate with the physician to set the medical plan of care.
• Establish and maintain a therapeutic relationship with patients and caregivers.
• Communicate effectively with the IDG, team, and families in writing and verbally.
• Document care outcomes and provide dietary counseling as indicated.
• Assume on-call responsibilities as scheduled.

Job Summary

Manage the over-all plan of care for a case load of hospice patients in collaboration with other members of the interdisciplinary group (IDG). Assess and identify patient/caregiver problems, establish nursing goals, and interventions. Work with physician to establish medical plan of care for the patient. Establish and maintain therapeutic relationship with patient and caregiver, including providing emotional support and teaching. Work closely with other members of the interdisciplinary group and communicates effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals. Document the outcome of care. Provides dietary counseling for patient care needs as indicated. Assume on call responsibilities as scheduled.

Requirements

• Graduate of an accredited School of Nursing
• Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
• Experience in an acute, hospice, home health setting, or long- term care setting a plus!
• CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
• Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
