Job summary

AuthoraCare Collective in Burlington, NC is seeking a Hospice RN Home Care Case Manager for the Burlington service area. This is a full-time position (32 hours per week / 4 days). CPR certification is required.

You will manage the overall care plan for hospice patients, assess problems, set goals with the IDG, and work with physicians to implement the medical plan. On-call responsibilities apply and you will document outcomes and provide caregiver education.

Qualifications

• Graduate of an accredited School of Nursing.
• Must be licensed as an RN in North Carolina and in good standing with NCBON.
• CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.
• Experience in acute, hospice, home health or long-term care is a plus.

Responsibilities

• Manage the overall plan of care for a caseload of hospice patients in collaboration with the interdisciplinary group (IDG).
• Assess and identify patient/caregiver problems, establish nursing goals and interventions.
• Work with physician to establish medical plan of care and document outcomes.
• Establish therapeutic relationships with patients and caregivers, providing emotional support and education.
• Participate in on-call responsibilities as scheduled.

Job Summary:

Manage the over-all plan of care for a case load of hospice patients in collaboration with other members of the interdisciplinary group (IDG) . Assess and identify patient/caregiver problems, establish nursing goals, and interventions. Work with physician to establish medical plan of care for the patient.

Establish and maintain therapeutic relationship with patient and caregiver, including providing emotional support and teaching. Work closely with other members of the interdisciplinary group and communicates effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals. Document the outcome of care. Provides dietary counseling for patient care needs as indicated. Assume on call responsibilities as scheduled.

Requirements

Education/Experience:

• Graduate of an accredited School of Nursing
• Must be currently licensed as an RN in North Carolina and in good standings with NCBON.
• Experience in an acute, hospice, home health setting, or long-term care setting a plus!

CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS) is required.

Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
